Log an event as soon as you know something happened. You can add people, documents, and follow-up afterwards.

Start from Events

  1. Go to Events.

  2. Choose the site in the top bar if you are logging for one site. Log event stays unavailable until an organisation is selected.

  3. Open Log event and pick the type.

  4. Fill in the form and choose Log event again to save.

Start from Work

On Work, open Add and choose:

  • Log accident

  • Log near miss

  • Log other event

Those shortcuts are shown only when Events is enabled for your organisation.

What to fill in

The form changes with the type. These fields appear on most events:

  • Site — often filled from the site you already have selected. The placeholder is Select site (optional) when you still need to choose one.

  • Title — required. ASR suggests a title from the type and date; edit it if you want a clearer name.

  • Date & time event occurred — required. You cannot pick a time in the future. A fire drill asks for Date & time of drill instead.

  • What happened? — required. Write what you know now, not a finished investigation.

  • What immediate action was taken? — optional, on types that ask for it.

  • Mark as important — flags the event so it stands out in the list. You can also flag it later. See How to find and filter events.

For Incident, also choose Incident type (for example Illness & outbreaks or Property & equipment damage).

Accident and near miss forms also ask who reported it: category, name, and contact details where you have them.

After you save

The event opens on its own page with status Open. From there you add affected people, Case notes, Documents, and actions. See How to add actions, notes and documents to an event.

Logging an event also creates a diary note for that day, so it shows on Diary as well as on Events.
