Use this when you need a new operational risk assessment that isn't already coming from an organisation template.
Start from the list
Go to Standards > Risk Assessments.
Choose New assessment.
The dialog is titled Start a risk assessment, described as Add a title, category, and scope, then continue in the guided editor.
Fill in the form
Title — required. Placeholder: e.g. Kitchen hot oil handling.
Category — optional. Choose a category, or leave No category.
Description — optional. Placeholder: What activities, areas, or equipment does this assessment cover?
Finish with Create and continue (or Creating... while it saves). Cancel closes without creating.
You're taken straight into the guided editor, starting on Hazards & scores.
Site context
If you're looking at a specific site in the top bar, the new assessment is usually scoped to that site. Site managers need a site selected before New assessment is available.
Organisation-wide templates are typically created and approved by admins rather than started as a one-off from this dialog — see How organisation templates are approved and sent to sites.
If you can't create
New assessment is hidden for viewers, and for members who can't create assessments.
Site managers won't see it (or can't use it) until a site is selected.
If the whole Risk Assessments menu is missing, the feature isn't enabled for your organisation.
What to do next
Work through the wizard steps — hazards, controls, residual scores, then summary — as described in How to complete the risk assessment steps. Publishing comes later; autosave keeps your draft as you go.
