How to record an issue and raise an action from a checklist

Flag a problem on a checklist task, then either record the fix you made or raise an action for someone else.

Checklists are most useful when they capture the things that aren't right. This article covers how to record a problem and what to do about it.

Record the issue

How you flag a problem depends on the task type:

  • Yes / No or Pass / Fail — choose No or Fail on the task row. The task opens so you can add detail.

  • Numeric — enter the reading, then tap Fail.

  • Free text — expand the task, describe what you found, then choose Mark Issue Found.

The first time you flag an issue, ASR asks you to confirm with "Mark this task as Issue Found?" — choose Continue. Tick Don't show this again if you don't want the reminder in future.

Always add a Comment describing what you found. This is what your manager, and any auditor, will read later.

Then choose what happens next

Every issue needs an outcome. You have two options.

Option 1: you fixed it yourself

Use this when the problem was dealt with there and then — a spill mopped up, a light bulb replaced, a door closed.

  1. Expand the flagged task.

  2. Type what you did into Corrective action comment.

  3. Tick Corrective action taken.

ASR records your name and the date against the fix, and you'll see an Issue resolved confirmation.

Option 2: someone else needs to deal with it

Use this when the problem can't be fixed on the spot — a repair is needed, a part has to be ordered, or it's someone else's responsibility.

  1. Expand the flagged task and choose Raise Action.

  2. Give the action a Title — short and specific, for example "Replace cracked tile in prep area".

  3. Add a Description (optional) with any extra context.

  4. Pick the Action Owner — the person who needs to do it.

  5. Set a Due date (optional).

  6. Choose Raise action.

You'll see Action raised, and the action now appears on the task itself with its owner, due date and status. The owner is notified and the action shows up under Work > Actions.

Closing the loop

Once a raised action has been dealt with, it can be closed from the flagged task on the checklist, or from Work > Actions. Until then it stays open so nothing gets forgotten.

Can you still submit a checklist with issues on it?

Yes. Recording an issue completes the task — it doesn't block you. When you submit, you'll see a note saying how many tasks were flagged, and the checklist is recorded as Completed with issues so it's easy to spot later.

Use the With issues filter on the Checklists page to review everything that has been flagged across your sites.

Where issues end up

Everything you record is kept as evidence:

  • On the checklist itself, against the task.

  • In the Diary, grouped under the task, with the corrective action or linked action shown.

  • In the Download completed report PDF, which lists exceptions, corrective actions taken and actions raised.
